Each nomination must be accompanied by two statements of support (from someone other than the nominator) which describes how the nominee fulfills the qualifications of the selected category. Complete the form below for the nominee. All fields are required in order to process the statement of support. (Please do not submit more than two statements of support or one nomination form for each nominee.) Do not submit your statement of support in an alternate format (email, fax, mail) after submitting this form online. Submit this statement of support no later than 5 pm on March 15, 2012. Two individuals must submit a statement of support to accompany the nomination form for the nomination to be considered. Submit a separate form for each nominee and each category.
The Review Committee reserves the right to re-assign the review category for the submitted nomination based upon the content of the nomination.
